Are You a Member or a Former Member of a 12-Step Fellowship?

Complete an online survey and enter for a chance to win an Amazon.com gift card!


If you are a member of AA or NA or if you were a member in the last year, you are invited to participate in a 25-40 minute online survey about your experiences as a 12-step member. The purpose of this study is to gather information to inform he

althcare providers about individuals in 12-step fellowships. It is hoped that results will improve treatment for individuals addicted to substances and to enhance referrals to 12-step programs. By participating, you will be have a chance to win an Amazon.com gift card worth up to $50. 

This survey is ANONYMOUS. You are not asked to provide personal identity information to participate in this study. You will be asked to provide a fictitious first name (pseudonym) and an e-mail address in order to enter the lottery for an Amazon.com gift card. 

This research study is being conducted by Amy Wytiaz, M.S., a psychology graduate student at Palo Alto University, under the direction of Louis Moffett, Ph.D. This study is not affiliated in any way with AA, NA, or any other 12-step organization.
